Why do I need to confirm my child’s evaluation appointment?

Confirming evaluations helps CTS keep schedules accurate and allows other families to access open appointments if needed.

 

When will CTS contact me to confirm the evaluation?

A CTS team member will contact you one week before your child’s scheduled evaluation.

 

How do I confirm the evaluation appointment?

You may confirm in one of the following ways within 2 business days of the confirmation call:

  • Provide verbal confirmation during the phone call

  • Reply in writing to the confirmation text message

  • Leave a voicemail on the new patient coordinator’s extension
    (Voicemails left on other extensions may be missed.)

 

Do intake forms need to be completed to confirm the evaluation?

Yes. All intake forms must be completed before the evaluation is considered fully confirmed.

 

What happens if I don’t confirm by the deadline?

If confirmation is not received by the deadline, your child’s evaluation will be canceled and the appointment will be offered to another family.

 

What happens if intake forms are not completed on time?

If intake forms are incomplete by the deadline, your child’s evaluation may need to be rescheduled.

 

What if I need to cancel or reschedule my evaluation?

  • Cancellations or reschedules made with less than 24 hours’ notice may result in a $50 fee.

  • You are responsible for contacting the new patient coordinator to reschedule for the next available appointment.

 

What happens if I do not show up and do not notify CTS?

Failure to notify CTS may result in:

  • A $75 no-show fee

  • Your child’s profile being moved to inactive status

  • Cancellation of all upcoming evaluations

  • A 6-month probation period during which new evaluations cannot be scheduled

CTS will also notify your child’s referring provider, and you will be responsible for obtaining new referrals after the probation period ends.

 

What should I do if I have an emergency or extenuating circumstance?

Please contact the office as soon as possible. CTS understands that emergencies happen and will review extenuating circumstances on a case-by-case basis.
