Evaluation Confirmation

Quick Guide

 

To keep evaluations running smoothly, CTS requires confirmation of your child’s evaluation appointment.

 

What to Expect

  • A CTS team member will contact you one week before the evaluation.

  • You must confirm the appointment within 2 business days by:

    • Confirming during the phone call

    • Replying to the confirmation text message

    • Leaving a voicemail for the new patient coordinator (other extensions may be missed)

  • All intake forms must be completed for the evaluation to be fully confirmed.

 

If Confirmation Is Not Received

  • The evaluation will be canceled and offered to another family.

 

Fees & Consequences

  • Cancelling or rescheduling with less than 24 hours’ notice may result in a $50 fee.

  • Not showing up or failing to notify the office may result in a $75 fee, account inactivation, cancellation of future evaluations, and a 6-month probation period before new evaluations can be scheduled.
